- Remember NOTARIZED COPIES of a Birth document are not accepted!

- Insert Consent for Treatment Forms
- You can print out Treatment Forms on the Babe Ruth League website!

- TEAM PHOTO
- Place copies in your book of the team photo, as well as have it prepared
to be sent electronically to make it easier for the host league to
assemble program books at the tournament.
